There are 3 plan options for your My Personal Guardian (MPG). Each option determines the amount of participation you want to have in collecting and determining what should be placed on your MPG.

GOLD
This plan is perfect for those with computer skills. You can complete all the information gathering documents on-line and mail us a few documents. This will get you your "My Personal Guardian" data device quickly. (Not yet available)

PLATINUM
This plan is great for those without computer skills. You will receive a Customer Fulfillment Kit which includes all the documents you need to fill out so that we can get your "My Personal Guardian" data device to you quickly.

PLATINUM PLUS
This option is our most popular plan since it requires the least amount of work for you. You will receive a Customer Fulfillment Kit which includes all the standard documents from our other plans PLUS the release forms so that WE can contact your doctors directly and gather all your records and place them on your "My Personal Guardian" data device. This option take longer but will include actual documents and reports from your physicians.

Each Customer Fulfillment Kit includes:
Welcome Letter, Customer ID Card, Key Ring, Fridge Magnet, Window Display Stickers, Personal Health Questionnaire, Referral Letter, HIPAA and Legal Disclosures.

You must purchase one of the Basic plans and update plans are an additional charge.
